On bank 2 the long-term correction adjusts fuel quantity from the oxygen sensor reading. At its lower limit, meaning the mixture is too rich, the ECM has already reached its limit and cannot cut the fuel any further.

The injectors are leaking
The mass air flow sensor is broken
The engine oil is contaminated
The oil stands above the max level
