Technical Description
The P0191 code is stored when the Engine Control Unit (ECU) detects that the signal from the Fuel Rail Pressure (FRP) sensor is outside the expected operational range or shows erratic performance. The ECU relies on this sensor to accurately manage fuel delivery. If the voltage signal fluctuates beyond calibrated thresholds or does not correlate with the fuel pump command (PWM signal), the ECU triggers P0191. This often affects the fuel trim strategy and can force the vehicle into a reduced power mode (Limp Mode) to prevent mechanical damage to the high-pressure system.
Possible Causes
Faulty Fuel Rail Pressure (FRP) sensor (internal failure or clogged sensor port)
Wiring harness issues (corroded connectors or frayed signal wires)
Faulty fuel pressure regulator or leaking fuel injectors causing pressure drops
Failing fuel pump (lift pump or high-pressure pump)
Very rare: ECU software issue or severely clogged fuel filter

- Perform a visual inspection of the sensor connector for corrosion (green crust) or backed-out pins.
- Inspect the fuel filter; a restricted filter can cause pressure fluctuations that trigger P0191.
- Use a multimeter to verify the sensor's 5.0V reference signal, ground connection, and signal voltage (typically 0.5V at key-on-engine-off and between 1.2V to 4.5V during operation).
- Use a scan tool to monitor 'Desired' vs. 'Actual' fuel pressure. If they deviate significantly despite a new sensor, suspect the regulator or high-pressure pump.
- Replace the Fuel Rail Pressure sensor if internal failure is confirmed.
