P0295 is a generic diagnostic trouble code (DTC) indicating that the Engine Control Module (ECM) has detected a high voltage condition in the fuel injector circuit for cylinder 12. This typically signifies a short circuit to voltage or a high resistance/faulty internal solenoid in the injector itself. The ECM monitors the current and voltage drop across the injector circuit; if it detects that the voltage remains high when the injector should be pulsed low, it sets this code to protect the circuit and maintain emissions compliance.
