This indicates that the fuel injector for cylinder 4 is likely stuck in an open position or leaking, causing an excessive amount of fuel to enter the combustion chamber. The ECU monitors crankshaft speed fluctuations and oxygen sensor feedback to isolate the fault to the specific cylinder.

Faulty or dirty fuel injector on cylinder 4 (mechanical leak)
Wiring issues (short circuit to ground on the signal wire keeping the injector open)
Faulty ignition coil or spark plug on cylinder 4 (unburned fuel interpreted as rich condition)
Internal ECU failure (shorted injector driver circuit)
Mechanical engine fault (e.g., poor compression on cylinder 4)
