Technical Description
P0040 is a diagnostic trouble code (DTC) indicating that the Engine Control Unit (ECU) has detected that the upstream oxygen (O2) sensor connectors for Bank 1 and Bank 2 are swapped. The ECU monitors air-fuel ratio corrections; when it adjusts fuel delivery for Bank 1 but sees the corresponding voltage change on the Bank 2 sensor input, it determines the wiring or sensors are cross-connected. This leads to major fuel trim errors as the ECU attempts to lean out or enrich the wrong cylinder bank.
Possible Causes
Oxygen sensor connectors for Bank 1 Sensor 1 and Bank 2 Sensor 1 are physically swapped during installation.
Wiring harness damage or incorrect repair crossing the signal wires.
Incorrect pin-out in the engine wiring harness or at the ECU connector.
Internal ECU software fault or incorrect calibration mapping.
